What We Do
The Vaughan Community Health Centre (VCHC) is non-profit, community-governed organization funded by Ontario Health and the Ministry of Health. VCHC provides primary health care, chronic disease management, health promotion, illness prevention and community-based programs to meet the needs of Northern York Region and Western York Region residents using an interdisciplinary team-based approach. In particular, we serve those people experiencing barriers to accessing health care services due to culture, language, age, chronic illness, or the like. As part of the health promotion framework of our services, we work in partnership with other community-based health and social service organizations to address individual, family, and community needs.

Feedback
The Vaughan Community Health Centre strives to provide our clients and community members with supportive and person-centered care. Suggestions, praise, or constructive feedback can help us to improve. The VCHC has an open door policy and welcomes feedback and comments from our clients and visitors. The VCHC Leadership Team will be happy to meet with you in person and discuss how we can be of further assistance to you. We appreciate and welcome all feedback.
Feedback is welcome in any format: in person, by phone, or in writing.
- VCHC will also solicit input from clients on an ongoing basis throughout the year by conducting a client experience survey.
- VCHC provides suggestion boxes in waiting and program areas to receive written comments and suggestions.
- Verbal feedback will be documented, by clients or staff, on a comment card and placed in one of the suggestion boxes.
- Input from community agencies, leaders, members, and other stakeholders will be gathered every 3 or 4 years when conducting the environmental scan for strategic planning.

Multi-Sector Service Accountability Agreement (MSAA)
The Vaughan Community Health Centre has a Multi-Sector Service Accountability Agreement (MSAA) with the Ontario Health. Both parties are committed to working together with local communities to achieve provincial priorities in building a connected and sustainable health care system centred around the needs of clients and their families. Quality improvement plan
Annual quality improvement plans guide the Vaughan Community Health Centre in its efforts to deliver the highest quality of services to its clients. The Quality Improvement Plans are approved and monitored by the VCHC Board of Directors. The Quality Improvement Committee comprised of Leadership Team, primary care and allied health professionals, and community representatives, monitor the progress of the plan on a quarterly basis.
