Nutrition
Our nutrition counselling services are provided by Registered Dietitians. We give priority for services to individuals from equity deserving communities, those living with complex medical and social needs, chronic medical conditions, and those facing barriers related to social determinants of health.
Our team supports non-team-based primary care providers by accepting referrals for their vulnerable (medically and socially complex) clients to services available at VCHC. Please find our referral form and fax number below. We also accept self-referrals.
Services
Services Included:
- Education on food allergies and intolerances.
- Prenatal, infant, toddler nutrition.
- Hypertension, dyslipidemia, meal planning for digestive issues (e.g. reflux, IBS, IBD), weight management.
- Engaging cooking and nutrition education group programs.
Services are available at Vaughan and Keswick sites.
