System Navigators support community residents with navigating the social, health, legal and other systems. System Navigators support community residents with needs related to social determinants of health such as employment, income, housing, completion of government forms and more.
Our team supports non-team-based primary care providers by accepting referrals for their vulnerable (medically and socially complex) clients to services available at VCHC. Please find our referral form and fax number below. We also accept self-referrals.

- Referral and connection to appropriate organizations and government services.
- Help with completing applications and forms.
- Support in navigating, understanding and accessing various systems such as:
Education, employment and volunteering, financial or social assistance (including subsidies), health, home and community care, housing, newcomer settlement and more.
