Diabetes Education
Living with Diabetes can be challenging but with the right knowledge and support you can take control of your health. Controlling blood sugar is an important part of diabetes self-management. What you eat, your physical activity, and how you manage stress can all affect your blood sugar levels. We serve individuals 18 years of age and older who are at risk or living with Type 2 Diabetes Our services are provided by Registered Nurse Educators and Dietitians.
Services
Individual
Individual appointments tailored to client needs and provide an opportunity to learn more about diabetes management, including:
- Preventing diabetes complications
- Oral medications and insulin therapy
- Creating a personalized meal plan
- Developing and activity routine
- Monitoring your blood sugar levels
- Recognizing symptoms of high blood sugar and low blood sugar and how to treat them
Group
Group sessions help clients get basic and more advanced knowledge about diabetes and provide an opportunity to learn from other people’s experiences. For more information about upcoming group programs, please visit our program calendar page. Topics discussed during these sessions include:
- Pre-diabetes and type 2 diabetes
- Prevention of long-term diabetes complications
- Healthy eating and meal planning
- Weight management
- Exercise and goal setting
- Glucometer training
