At Vaughan Community Health Centre (VCHC), we provide timely and appropriate primary care services to our clients by allowing clients to book appointments when needed. The VCHC is mandated to serve vulnerable populations facing health inequities created by barriers in accessing health care.
Our primary care team includes Family Doctors, Nurse Practitioners, a Registered Practical Nurse and Phlebotomist. The team provides all primary care services including:
- Prenatal and postnatal care
- Medical checkups
- Care for chronic diseases
- Treatment of illnesses
- Blood work requested by VCHC provider
- And more
Important information for clients registered with a VCHC doctor or nurse practitioner:
- Please call VCHC to notify your provider that you have been discharged from the hospital. Your Family Doctor or Nurse Practitioner would like to reach out to you within 7 days after discharge from hospital.
- Please bring ALL of your medication to each appointment visit.
- When coming in for an appointment, please start with the most URGENT concern first.
- VCHC does not accept external laboratory requisitions from external doctors (such as: specialists, walk in clinics, etc.)

Can I go to another general practitioner while I am a client of the Health Centre?
As per our model of care, clients who agree to register to see a physician or nurse practitioner at the Vaughan Community Health Centre must see only the VCHC physician or nurse practitioner for their primary health care needs. There are exceptions when clients visit a walk-in clinic for urgent issues because the VCHC is closed. In that case, please be sure to inform your physician or nurse practitioner. Double-doctoring is not accepted at VCHC. During registration, clients are asked to complete a release of medical information form to transfer medical records from their previous provider to the VCHC.

What happens if I do not visit my primary health care provider for a long time?
The VCHC will close the medical record of registered clients who do not access primary healthcare services for three consecutive years. Clients are responsible for updating changes in their personal health information e.g. phone number and address, etc.

I received a call from VCHC why don’t they leave a detailed phone message?
To maintain the privacy of your health information, the VCHC will not leave detailed information on voicemail messages.

Why am I asked the reason for my visit when I call to book an appointment?
This is to ensure that enough time is booked for you to see the provider. Please provide only general information for the visit. If you do not wish to disclose the reason, then please inform reception staff.

How do I reach a doctor after regular VCHC hours of operations?
If you are registered with VCHC Primary Care Team and require access to a doctor before or after clinic hours, a physician can be reached by phoning our main line (905) 303-8490. Please note that the physician on-call may not be your healthcare provider at VCHC.
For any emergency, please call 911 for assistance. You should call 911 even if you do not speak English. They will provide interpretation and send help to address your emergency.

How do I refill my prescription?
When you are running low on your medication, please ask your pharmacy to fax the prescription refill request to 905-832-0093.
A provider will review the request within 3 working days. You will be notified if a follow-up appointment is required.

How do I know when my appointment is booked with a specialist?
When a referral is made by VCHC providers to a specialist, the reception team will book the appointment and notify you of the appointment date. Some specialists may call you directly with the appointment date, so remember to call VCHC to let us know of your appointment date, for our records.
It is very important that you keep the appointment date with the specialist. You are responsible for notifying the specialist if you need to change or cancel the scheduled appointment date and inform the VCHC of the change.
Some specialists may charge a fee for no show or cancellation.
Our providers are not able to make referrals for 2nd or 3rd opinion.

I have a lab requisition from another doctor, can I book an appointment with VCHC?
Due to limited resources, the VCHC does not accept external lab requisitions from external doctors (such as: specialists, walk-in clinics, etc.)
Please see our reception staff for a list of local labs.

I received a letter to do cancer screening test what should I do?
Clients may receive letters from Ontario Cancer Screening Program as a reminder to do testing or follow-up with results. When you receive the letter, please call the VCHC to book an appointment with your provider.

ConnectingOntario is a province-wide information sharing system for providers to access personal health information on demand. VCHC is working with ConnectingOntario to ensure that our providers have access to updated information so that we can provide you with accurate and effective treatment and care. Your privacy is a priority for both VCHC and ConnectingOntario. ConnectingOntario ensures that access to your medical records is limited to only providers involved in your care.
