The Vaughan Community Health Centre Keswick Site is currently offering the People Accessing Care Teams services and health promotion programs.
People Accessing Care Teams (PACT) Services:
PACT provides Medical Doctors and Nurse Practitioners who do not work in a team-based model with access to an inter-professional care team for their clients/patients.
PACT Services Include:
- Case Management
- Mental Health (social work and psychology)
- Chiropody
- Nutrition Counselling
- Physiotherapy
- Lung health/ Spirometry testing
- Smoking Cessation
- Social Programs & Health Promotion
Accessing PACT services:
PACT serves individuals with medical and social complexities.
To Access Northern York Region PACT, you must:
- Be an individual who does not have an open case with the WSIB or have an open case related to a motor vehicle accident;
- Have a lack of extended health benefits, or lack of health coverage (NO OHIP);
- Experience any of the following: have a low- or moderate-income; inadequate housing, unemployment, lack of transportation to medical appointments; be unable to speak English or French; and/or have complex health issues.
How to Access PACT:
To access the PACT program, please ask your physician or nurse practitioner to submit your referral form to our office.

PACT providers are located across Northern York Region. In the Northern York Region office, PACT services are offered in Keswick, Georgina. All clients that access the PACT program must attend their first appointment with our Systems Navigator Case Manager at our Keswick office.

Health Promotion Programs:
The PACT Northern York Region clients have access to all the health promotion programs offered by Vaughan Community Health Centre. The health promotion programs focus on mental, social and physical wellbeing. Currently, due to the COVID-19 Pandemic all group programs are held online for the safety of the community.
Our Social Programs Focus On:
- Health awareness presentations
- Healthy nutrition
- Mental health & wellbeing
- Socialization and capacity building
- Support groups
